INTRODUCTION
The continued demand of improved patient care services and ensuring that patients are safe in the care environment implies that nurses should have adequate training. Apart from attending theory classes, nurses engage in other activities to better prepare them for better management of chronic diseases such as diabetes (Hockenberry & Wilson, 2018). One of such is practicum experience. Practicum or clinical experience helps baccalaureate-prepared nurse have a chance of contributing to an identified patients’ outcomes. As way of continued learning, the nurse can reflect on such experiences. One way of reflection is video reflection. As such, this presentation explores a reflection on the practicum experience. In addition, the presentation will also focus on a formulated intervention used in addressing a health problem impacting a chosen patient.
ASSESSMENT OF THE INTERVENTION CONTRIBUTION
The patient whom I had an experience with during my clinical hours is the Mr. Y. He is my neighbor and suffers from diabetes. He also has congestive heart failure as a comorbid condition. The patient was diagnosed with diabetes five years ago and has been treating it with medications, lifestyle, and behavioral modifications. Three months ago, he was also diagnosed with congestive heart failure and initiated treatment. The main evidence-based practice intervention applied in patient education. I have managed to educate the patient about lifestyle modifications and the importance of medication adherence. I also helped coordinate interprofessional collaboration for better health outcomes. Interprofessional collaboration is especially key in managing the patient’s comorbid conditions. Such interventions have been connected to improved quality of life.
PATIENT FEEDBACK AND PATIENT EXPERIENCE
One of the sources through which one can know about the impact of an implemented intervention is patient feedback (Hockenberry & Wilson, 2018). In most cases, patients give honest opinion on what they think regarding the intervention and how their lives have been impacted by the intervention in question. Mr Y gave a feedback on the intervention. Earlier, Mr Y shared some worrying statements regarding his condition. conditions have increased the healthcare costs that he incurs in treating them. He faces the challenge of increased need for frequent hospital visits, hospitalization, and the purchase of expensive medications. He has also lost his social and occupational productivity due to health problems. He is worried that the medications he currently uses may predispose him to further adverse health outcomes. The increased demands due to the conditions have affected the social, emotional, and psychological wellbeing of his family. However, the intervention positively impacted on the patient’s health. He was appreciative of the intervention and indicated improved medication adherence and better increased involvement in physical exercise and healthy diet.
USE OF PEER-REVIEWED LITERATURE AND EVIDENCE
The successful planning and implementation of a capstone project requires a lot of input and focus on various aspects of research. Central to such a process is the use of peer-reviewed literature and evidence (Chinn et al., 2021). Therefore these two were key in informing the plan and implementation of the capstone project. The use of literature review was key in offering a theoretical framework upon which the intervention was to be based. From literature, it was evident that nursing interventions require theoretical foundations. The evidence used was that the use of comprehensive educational initiatives have shown efficacy in addressing and managing various chronic conditions, so it was a foundation for argument that the intervention would help manage Mr. Y. the project was also guided by the principles of evidence-based practice. For example, that evidence used should come from four sources which include scientific findings, the organization, practitioners and stakeholders.
LEVARAGING HEALTHCARE TECHNOLOGY
In recent years, technology has widely been applied not only in healthcare but also in other sectors. Technologies that have been adopted for diabetes management include Telehealth, automated bolus calculators, continuous glucose monitoring, implantable continuous glucose monitors/sensors, integrated systems, smartphone, applications, and wearable technologies (Alcántara-Aragón, 2019).
