My project is about telephone-based post-hospital discharge follow-up to reduce the rates of early unplanned readmission among elderly patients. Several issues may arise during the implementation of this project. One of these is that the security and confidentiality of the patient information may be affected during telephone calls. Another issue is that some of the elderly patients may have hearing impairments making it a challenge for effective telephone communication.  In addition, nurses may not have adequate knowledge and skills to conduct follow-up calls, therefore requiring additional training, which may call for more resources.  To create a higher readiness level in the organization the staff will be trained and supported to boost their confidence and skills to conduct effective telephone follow-ups. In addition, the directly involved stakeholders (nurses and patients) will be encouraged to give their views and suggestions for effective change implementation.

Literature and current research supporting the use of telephone-based follow-ups post-discharge and evidence-based frameworks will be utilized to support the change initiative. For example, Chang et al (2023) show the effectiveness of telephone follow-ups in reducing unplanned readmission and Gabutti et al. (2023) provides an evidence-based framework for implementing change in a hospital. These alongside the Organizational Readiness to Change Assessment (ORCA) Tool will be used to support and ensure that the project is successfully implemented.
